THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice applies to Pique Aesthetics LLC d/b/a Pique Vitality, Dr. Lauren Saltzburg, D.O., MBA, MA, and the clinicians, contractors, administrative team members, and staff who work under her supervision or on behalf of the practice (“Pique Vitality,” “we,” “us,” or “our”).
How This Notice Is Different From Our Website Privacy Policy
This Notice of Privacy Practices explains how Pique Vitality may use and disclose your protected health information (“PHI”) and describes your rights under HIPAA. It is separate from, and in addition to, our website Privacy Policy. Our Privacy Policy explains how information is collected through our website and online tools — for example, browsing activity or contact form submissions. This Notice explains how your medical information may be used and disclosed in connection with your care, treatment, payment, healthcare operations, and other purposes permitted or required by law.
Our Commitment to Your Privacy
We are required by law to maintain the privacy of your protected health information, also known as “PHI,” to provide you with this Notice describing our legal duties and privacy practices regarding your PHI, and to follow the terms of the Notice currently in effect.
PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health condition, healthcare services, treatment, payment, or related healthcare operations.
Pique Vitality is a self-pay, physician-led concierge telehealth and wellness practice. We do not bill insurance, Medicare, or Medicaid. Your medical record is maintained electronically through secure practice-management and healthcare technology platforms.
How We May Use and Disclose Your Health Information
The following categories describe the ways we may use and disclose your PHI. Not every use or disclosure in a category is listed, but all permitted uses and disclosures fall within one of these categories.
Treatment
We may use and disclose your PHI to provide, coordinate, or manage your medical care. For example, Dr. Saltzburg may review your health history, lab results, medications, treatment plan, or clinical images to evaluate care options. We may also share information with pharmacies, laboratories, diagnostic partners, or other healthcare providers involved in your care.
Payment
We may use and disclose your PHI to bill and collect payment for services. Because Pique Vitality operates on a self-pay basis, this may include using your information to process payment directly with you through our payment platform, provide receipts, manage membership or consultation fees, or address billing questions. We do not submit claims to insurance, Medicare, or Medicaid.
Healthcare Operations
We may use and disclose your PHI to support the day-to-day operation of the practice. This may include quality review, staff training, internal documentation, scheduling, care coordination, patient support, compliance review, technology operations, business planning, and evaluating the services we provide.
Pique Vitality may use secure technology tools, including HIPAA-compliant clinical documentation, lab-interpretation, or decision-support tools, to assist with care coordination and documentation. No technology tool replaces physician judgment. Final clinical decisions and recommendations are made by the licensed medical provider.
Other Permitted or Required Uses and Disclosures
We may also use or disclose your PHI without your written authorization in certain circumstances permitted or required by law, including:
- As required by federal, state, or local law.
- For public health activities, such as reporting certain diseases, injuries, adverse events, or other matters required by law.
- To report suspected abuse, neglect, or domestic violence to appropriate authorities when required or permitted by law.
- For health oversight activities, such as audits, investigations, inspections, licensure, or regulatory review.
- In response to a valid court order, subpoena, discovery request, or other lawful legal process, when applicable requirements are met.
- To avert a serious and imminent threat to your health or safety or the health or safety of another person.
- To coroners, medical examiners, or funeral directors as necessary.
- For organ, eye, or tissue donation purposes, where applicable.
- To the U.S. Department of Health and Human Services to investigate or determine our compliance with HIPAA.
- For workers’ compensation or similar programs, if applicable and permitted by law.
- For specialized government functions or law enforcement purposes, when permitted or required by law.
Uses and Disclosures Requiring Your Written Authorization
Other than as described in this Notice or as otherwise permitted or required by law, we will not use or disclose your PHI without your written authorization.
Your written authorization is required for:
- Use of your name, photograph, video, image, voice, testimonial, treatment story, before-and-after image, or other identifiable information for marketing, website, social media, advertising, public relations, or promotional purposes.
- Most uses and disclosures of psychotherapy notes, where applicable.
- Sale of your PHI.
If you provide written authorization, you may revoke it in writing at any time, except to the extent we have already relied on the authorization.
Your Rights Regarding Your Health Information
You have the following rights regarding your PHI:
- Right to Inspect and Copy. You may request to inspect or obtain a copy of your medical record and other health information we maintain about you. We will respond to your request as required by law, generally within 30 days.
- Right to Request Amendment. You may ask us to amend your PHI if you believe it is incorrect or incomplete. We may deny the request in certain circumstances, but we will provide our reason in writing if we deny it.
- Right to an Accounting of Disclosures. You may request a list of certain disclosures we have made of your PHI. This accounting does not include disclosures made for treatment, payment, healthcare operations, disclosures made to you, disclosures made pursuant to your authorization, or certain other disclosures excluded by law.
- Right to Request Restrictions. You may ask us to restrict how we use or disclose your PHI for treatment, payment, or healthcare operations. We are not required to agree to every restriction request. However, if you pay out of pocket in full for a healthcare item or service and ask us not to disclose information about that item or service to a health plan, we will honor that request unless disclosure is required by law.
- Right to Request Confidential Communications. You may ask us to communicate with you about your health information in a specific way or at a specific location, such as a particular phone number, email address, mailing address, or patient portal.
- Right to a Paper Copy. You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
- Right to Notification of a Breach. You have the right to be notified if there is a breach of your unsecured PHI, as required by law.
Our Responsibilities
- Maintain the privacy of your PHI.
- Provide you with this Notice of our legal duties and privacy practices.
- Follow the terms of the Notice currently in effect.
- Notify you if there is a breach of your unsecured PHI, as required by law.
- Refrain from retaliating against you for filing a privacy complaint or exercising your privacy rights.
We reserve the right to change the terms of this Notice and to make the revised Notice effective for all PHI we maintain. If we make a material change, the revised Notice will be posted on our website prior to the effective date of the change.

If you believe your privacy rights have been violated, you may file a complaint with Pique Vitality using the contact information below.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
You will not be retaliated against for filing a complaint.
